## Instructions

Please complete all sections of the Assessment Support Request Form. Upon completion, submit your request to the Office of Assessment and Institutional Effectiveness by clicking on the Submit Request button. An OPAR staff member may contact you by phone or e-mail to discuss your request within 3-5 business days.

Please describe the purpose for request:

Examples: Analyzing Assessment Results, Developing Objectives, Assessment Tools, Aligning Budgets, Identifying Activities, Assistance with Entering Information into the Electronic Assessment System (Nuventive), and Open Assessment Days.
